Diabetes Self-Management Education Programs

  • Managing Type 2 Diabetes—Basic skills and knowledge in self-management of Type 2 diabetes. Classes available weekly in morning, afternoon and evening time periods.
  • Using Insulin—Practical guidelines for using insulin and basic pattern management techniques for evaluating blood sugar results. Class held monthly.
  • Carbohydrate Counting—Learning insulin to carbohydrate ratio or consistent intake of carbohydrates to manage blood sugars. Classes available weekly.
  • Insulin Pump Program—Four-step program to provide intensive education from basic to advanced training of insulin pump therapy.
  • Continuous Blood Glucose Sensor Program—Diagnostic tool used by health-care professionals to track continuous blood sugar patterns and make appropriate recommendations.
  • Gestational Diabetes—Meal plan, blood sugar monitoring and blood sugar goals for management of gestational diabetes. Classes available weekly
  • Diabetes Cooking Class—Lively cooking sessions based on nutritious recipes every quarter. Tasting required.
  • Diabetes Health Fair—Annual event for the community with an emphasis on self-management skills. Focus on fun and living healthy with diabetes. Held yearly in March.
  • Young Adult Program – Survival: Life on Your Own—Yearly program for young adults transitioning to self care. Focus on practical aspects of living with diabetes. Held yearly in July.
  • Diabetes Prevention Program—Focus on pre-diabetes and prevention of diabetes through education for healthy eating and exercise.
  • Exercise Testing and Prescription—Education and counseling with exercise physiologist. Appointments available Tuesday through Friday 8AM – 330PM
  • Pediatric Education—Individual education provided by both nurses and dieticians.
